Just wanted to update you. I was doing some troubleshooting yesterday with the form in the footer but thus far haven't been able to get to the bottom of what's going on there.
Could you tell me if the two JavaScript items (Use Javascript Support? and Use Javascript DatePicker?) under "List Options" on the plugin settings page are checked or unchecked? I suspect that there's some element of the plugin scripts and/or CSS which is conflicting with other elements on the site causing some weird behavior, so I'm working on pin-pointing that.
